Behavioral Health Acknowledgement Information Sharing with United Community Health Center (UCHC) Primary Care Provider.
United Community Health Center (UCHC) is a team of healthcare professionals committed to fulfilling the mission of delivering quality care that is Accessible, Comprehensive, Innovative, Compassionate and Culturally Sensitive in an Atmosphere of Trust and Respect. In order to achieve this mission, it is important to have communication between your Behavioral Health Providers and your Primary Care Providers (PCP).
The Health Insurance Portability and Accountability Act (HIPAA) allows healthcare providers to disclose protected health information (PHI) contained in your medical records with other health care providers. Mental health information is considered the same as any other health information. HIPAA generally does not limit disclosures of PHI between healthcare providers for treatment, case management, and care coordination. The only exception is psychotherapy session notes; if you would like this information to be shared with your UCHC Provider, you will be asked to complete an Authorization To Release Health Information Form. (See 45 CFR 164.501).
Please Note: Your UCHC healthcare providers will only share your information with others involved in your care at UCHC and if your provider determines, based on their professional judgment, that doing so is in your best interest.
